First Visit

Here at The Basil Center, we pride ourselves in our ability to keep you informed about your dental care so you can make the best decision regarding YOUR dental needs. Your initial appointment will begin with a consultation with Dr. Basil to get to know a little about you and alleviate any fears or concerns you may have.  Your complete dental and medical history will be reviewed and a thorough exam completed that includes:

  • A Full Mouth Exam
  • Periodontal exam
  • Oral cancer exam
  • TMJ/Occlusal/ bite exam
  • Teeth Cleaning
  • X-rays

Dr. Basil will review the findings, sit down with you to discuss the different treatment options and also take time to listen to your concerns, needs, and desires.  He feels it is important that his patients know what THEY can do to prevent dental disease themselves, thus maintaining a healthier smile. Dr. Basil will also answer any questions and help you to determine the best course of action for your individual treatment plan.

 

 

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT:All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
